    Terms

    This Gas Fireplace & Gas Log Annual Service Plan Agreement (“Agreement”) is entered into between The Fireplace Shop at Henry Poor Lumber (“Service Provider”) and the  undersigned customer (“Customer”).

    Plan Overview

    This plan provides routine inspection, cleaning, and operational servicing of residential gas fireplaces
    and vented or vent-free gas log sets. Wood-burning fireplaces and chimney sweeping are excluded.

    Services Included

    • Safety inspection of firebox, glass, gasket, and seals
    • Inspection of venting systems (direct vent, B-vent, or approved vent-free configurations)
    • Cleaning of burner, pilot assembly, and gas logs
    • Ignition system verification (millivolt, IPI, or IFT)
    • Flame pattern evaluation and minor adjustments
    • Blower inspection (if equipped)
    • Remote, switch, or thermostat testing
    • Written service report with findings and recommendations

    Services Not Included

    Replacement parts, repairs beyond minor adjustments, glass or gasket replacement, vent repairs,
    electrical repairs beyond basic testing, warranty labor, emergency or after-hours service, or chimney
    sweeping.

    Pricing

    $199 per gas fireplace or gas log set per year
    (Standard single-visit service: $225)

    Payment Terms

    Payment is due in full at enrollment. This plan is valid for twelve (12) months from the date of purchase.
    We will contact you upon completion of this form.

    Renewal & Cancellation

    Plan will automatically renew annually unless cancelled prior to the annual service visit. Once the
    annual service visit has been completed, the plan is considered fulfilled and non-refundable.
